SKILLED NURSING
Licensed nurse assesses and monitors patients’ health care needs, educates patients/caregivers, and collaborates with physicians to promote continuity of care.
PHYSICAL THERAPY
Licensed therapist helps patients regain functional mobility and gross motor skills through graded therapeutic exercises, balance activities, and ambulation training.
OCCUPATIONAL THERAPY
Licensed therapist helps patients regain daily living skills with the use of cognitive, perceptual, fine motor, and sensory exercises.
MEDICAL SOCIAL WORK
Medical Social Worker evaluates social and emotional factors affecting the patient and connects patients/families with necessary community resources.
HOME HEALTH AIDE
HHA assists patients with ADLs/personal care. Works under direct supervision of an RN.
REGISTERED DIETITIAN
RD is responsible for providing an evaluation, consultation, and instruction regarding appropriate nutrition for clients. The dietitian also can recommend special nutritional interventions to the physician’s Plan of Care in meeting the therapeutic needs of the patient.
